What are the permanent methods birth control?

Permanent Methods are:

1)Male: 99.85% vasectomy
2)Female: 99.5% tubal ligation
3)Essure: intra-fallopian tube coils that are inserted trans-cervically and induce scarring/closure
not covered by MSI, it is not possible for all patients, it requires dye test to confirm
